Waiver Agreement

I (Owner) represent that I am the legal owner of the above-named dog(s) and I take/assume all risks, dangers, and responsibility for injuries to the named dog(s). Owner understands and agrees that Owner is solely responsible for any harm while Owner’s dog(s) is/are attending Cherry Hogg’s daycare / boarding.

PHOTO AND VIDEO RELEASE
We love to post pictures and videos on Instagram, Facebook, and our website. Owner agrees to allow Cherry Hoggs to use Owner’s pet’s name and any images of Owner’s pet taken while he/she is at Cherry Hoggs, in any form, for use at any time, in any media, marketing, advertising, illustration, trade or promotional materials without compensation, and Owner releases to Cherry Hoggs all rights that Owner may possess or claim to such image, likeness, recording, etc.

DOG’S HEALTH
Owner further understands and agrees that Owner’s dog(s) are always healthy and well while attending Cherry Hoggs and have current vaccinations for rabies. Owner is not enrolling any dog that has any condition that could potentially jeopardize the health of our other dogs or people and has not had any potentially communicable condition within 30 days prior to enrollment. Owner further understands that even if Owner(s) dog is vaccinated for Bordetella (Kennel Cough) there is a chance that the Owner’s dog can still contract Kennel Cough. Owner agrees that Owner will not hold Cherry Hoggs responsible if Owner’s dog(s) contracts Kennel Cough or other dog-dog transmitted ailments.

GROUP PLAY
Owner further understands and agrees that in admitting Owner’s dog(s) to our doggy daycare, Owner is representing to Cherry Hoggs that Owner’s dog(s) is/are in good health and have not harmed, shown aggression, or exhibited any threatening behavior towards any person or other dogs. Any dog demonstrating aggression or other behavior deemed unacceptable by Cherry Hoggs staff may be required to leave daycare/boarding.

RELEASE OF LIABILITY
Owner understands and agrees that during normal dog play, Owner’s dog may sustain injuries. Dog play is always monitored by Cherry Hoggs staff to best avoid injury, but scratches, punctures, torn ligaments and other injuries may occur despite the best supervision. Owner further understands and agrees that neither daycare/boarding nor any of its employees, staff, or volunteers will be liable for any illness, injury, death, and/or escape of Owner’s dog(s) provided that reasonable care and precautions are followed, and Owner hereby releases Cherry Hoggs staff or volunteers of any liability of any kind whatsoever arising from or as a result of Owner’s dog(s) attending Cherry Hoggs.

PACK WALKS
Owner agrees to allow dog(s) to be taken to an off-leash dog park for pack walks. Owner further understands and agrees that any of Cherry Hoggs employees, staff or volunteers will not be liable for any illness, injury, death, and/or escape of Owner’s dog(s). Owner hereby releases Cherry Hoggs staff or volunteers of any liability of any kind whatsoever arising from or as a result of Owner’s dog(s) attending Cherry Hoggs off leash pack walk.

PAYMENTS
Owner agrees payment is due at the end of each week (Friday) and payable at the beginning of each week (Monday). Payment will be made through PayPal.

CANCELLATION / NO-SHOW POLICY
Owner understands and agrees any changes after 8am Monday will be charged.

HOURS OF OPERATION
Owner agrees to drop off dog no earlier than 8am and pick up their dog no later than 5pm and understands that a late fee of $10 every half hour will be incurred for any dog not picked up by the time.

TRANSPORT
Owner and agrees and understands that the transportation of animals may be dangerous and assume all the risks of using transportation services offered by Cherry Hoggs. I understand that Cherry Hoggs and/or their service providers will be providing transportation for my dog(s). I am aware of the possible risks and dangers associated with allowing Cherry Hoggs to transport my dog(s) to and from location(s) in transportation provided by commercial and/or private motor vehicles and I knowingly accept and assume all known and unknown risks related to the transportation of my dog(s) by Cherry Hoggs and my use of Cherry Hoggs dog transportation services, including without limitation all physical, mental, or health issues that may arise during or as a result of my dog’s transportation. Cherry Hoggs will not be responsible for any liability, such as stress, illness, injury, disability or death, and all property loss, damage, or theft.

Doggy Daycare, Dog Boarding Policies and Release of Liability
